antidromic studies

The antidromic is the propagation of a nerve impulse in the direction opposite to physiological. The antidromic nerve conduction studies are performed using a stimulus directed in the opposite way the nerve normally depolarizes, for example, applying proximal stimulation on a nerve course and recording responses more distally. The stimulation can be applied at several sites such as the palm, wrist, elbow or more proximally at the axilla and even at the supraclavicular level (Erb’s point). The sensory nerve action potentials (SNAPs) are so recorded distally at the wrist, on the palm or to the digit. The first antidromic studies were introduced in 1960s but mostly used since the 1980s. In clinical practice, the sensory antidromic nerve conduction studies have been gradually the preferred methods because of higher SNAPs amplitude than orthodromic sensory nerve conduction studies.
